Our SACOT program is designed for adults who need a higher level of care than traditional outpatient treatment but don’t require 24-hour residential services. This ASAM Level 2.5 program combines evidence-based therapies, holistic approaches, and experiential components to create a transformative healing experience.

Substance Abuse Comprehensive Outpatient Treatment (SACOT) is a specialized level of care within the addiction treatment spectrum defined by the American Society of Addiction Medicine (ASAM) as Level 2.5. Our SACOT program provides intensive, structured treatment while allowing clients to return home each evening, maintaining connections with family and practicing recovery skills in real-world settings.
SACOT bridges the gap between our Partial Hospitalization Program (PHP) and less intensive Intensive Outpatient Program (IOP), offering an important transitional level of care in the recovery journey.

Our location in Matthews, just outside Charlotte, North Carolina, provides a serene environment for healing while remaining accessible to clients throughout the region. The men’s property includes approximately 4 acres of fenced land with amenities such as a pool, outdoor grilling area, and gym, while our women’s house features beautiful modern accommodations with outdoor spaces for relaxation.

Dr. Neal Guffey is a dedicated physician with expertise in internal medicine, anesthesiology, hospice and palliative care, and addiction medicine. For over a decade, he has specialized in medications for addiction treatment (MAT), helping individuals and families navigate recovery with compassionate, patient-centered care. A graduate of UNC Chapel Hill and East Carolina University School of Medicine, Dr. Guffey is passionate about improving lives through evidence-based treatment. When he’s not in the office, he enjoys skiing, mountain biking, and landscaping, always seeking his next outdoor adventure.
Our SACOT program provides a significantly higher level of care than traditional outpatient treatment, with 20+ hours of programming weekly compared to just a few hours in standard outpatient care. Additionally, through our partnerships with transitional housing providers, we can offer an enhanced level of care that approaches the intensity of residential treatment while maintaining the flexibility of an outpatient program.
What truly distinguishes our program is our founder’s personal experience with recovery after attending 10+ treatment centers, our integrated approach to treating co-occurring disorders, our family-centered care model, our experiential weekend programming, and our partnerships with transitional housing that allow for 24/7 support. Our program was specifically designed to address the gaps our founders identified in traditional treatment models.
Many clients are able to maintain part-time work or school commitments while participating in SACOT. Our program schedule is designed to be intensive while still allowing for some flexibility. During your assessment, our team will discuss your specific situation and help determine if SACOT can accommodate your other responsibilities.

Families can either participate directly in the client’s treatment or engage in independent therapy with a clinician to begin their own healing process, ensuring the entire family system receives support.
Upon completion of SACOT, clients typically transition to our Intensive Outpatient Program (IOP) or other appropriate levels of care. Our clinical team works with each client to develop a comprehensive aftercare plan that supports continued recovery and addresses ongoing needs.
